Health Alert: Due to the Covid-19 pandemic the physical office is closed, telehealth is the new normal until safer at home guidelines have been lifted. The proposed date to return to the office is April 14th, 2020. Please keep checking back here for updates as this date could be extended based on CDC recommendations.
Directions for using the Tele Therapy option in this portal: When you are here and logged in to your patient portal from the home page select "TeleTherapy" then allow your camera to work. I will also be logged in on my side of this electronic health record, and join you. We would coordinate to meet in the portal at your appointment time, disable the audio in your portal, and I will call you. at the appointment time.
You can use any internet-connected device with a camera.

Patient Portal Log in directions for those who already have a patient portal account.
1: See the upper right-hand corner of this page (or if on mobile device upper left click on the 3 bars) *Must use the Google or Mozilla browser for the Sign-in or see below
2: (Enter) Username-----username is your first and last initial (lower case) followed by either the last 4 digits of your SSN (or) last 4 digits of your phone number. For example ab1234 This is only if we set your account up if you establish an account via the patient portal, most people use their email address.
3: (Enter) Password---your initial password is your date of birth (mm/dd/yyyy) with the "/" between the numbers. For example 08/12/1962 Please consider using your date of birth (mm/dd/yyyy) at least initially even if you are setting up your own account.
4: (Select) Sign In-> See under Clinical Documents: "New Patient Document Instructions" for more information about the use of this patient portal

Please DO NOT hit "HELP" as it will take you to another general site with the electronic health record, but not with TCCS (Take Charge Counseling Services) and we cannot access that "Monitoring" account.
