Directions for using the Tele Therapy option in this portal: When you are here and logged in to your patient portal from the home page select "TeleTherapy" then allow your camera to work. I will also be logged in on my side of this electronic health record, and join you. We would coordinate to meet in the portal at your appointment time, if there is a lag in video please disable the audio in your portal, and I will call you.

If you do not have a patient portal account please email firstname.lastname@example.org with your date of birth I will set your account up with all the new patient documents so you can log in to complete these forms and select a telehealth appointment. I will review your new patient forms and decide if I am a good match for you.
If you have an account please log in (using either the Google or Mozilla browser) to your secure patient portal at tccs.carepaths.com with the user name: either your first and last initial followed by the last 4 digits of your ssn, or your email and password: date of birth in the mm/dd/yyyy format to add additional security to protect your private health record it is recommended that you change your password once logged in.
Once logged in you can access my available times, but please complete the new or returning patient forms prior to selecting an appointment.
